All Baylor employees and spouses are NEW USERS and need to click on the Start Registration button below to establish a new username and password.
Once this is entered the first time, you then become a Returning User and will enter the username and password created upon future website access.
Baylor employees may also Single-Sign-On to the Thrive website directly from MyBaylor after the initial registration.
All Baylor employees and spouses must complete the Wellness Assessment online by November 30, 2013 to be eligible for 2014 Thrive rewards and avoid a surcharge if enrolled in 2014 Baylor medical benefits.
The Health Questionnaire gets an update, too, and is now known as the Wellness Assessment!
The Wellness Assessment contains new questions, scores (including your "Health Age"), and a more detailed report to help you understand your health success areas and areas for improvement. Take the Wellness Assessment after creating your New User account and logging into the system.

Want to make sure your 2014 Health Screening form was received? Log into the Thrive site and look for the CONGRATULATIONS notification at the top right area of the Home Page. If you have an Action Needed notification, please send your form to Thrive via fax at 214-820-8773 before the November 30, 2013 deadline.
*New Hires as of October 1, 2013 are not required to complete a 2014 Health Screening form
